A set of data on aspects of dementia published on the myhealthlondon website.

The publication by NHS London of the nationally agreed set of 25 indicators on dementia care meets the 5th commitment of the Prime Minister’s Dementia Challenge, ‘Promoting local information on dementia services’. The indicators gather data on local services in one place and cover the majority of the care pathway.

These indicators were first published as a set as part of the information offer on dementia pioneered by NHS South West in March 2012, via the website Our Health - South West.

DEM0001 - Number of patients with dementia recorded by GP practices.
DEM0002 - How effectively GP services are able to identify patients with dementia.
DEM0003 - Percentage of patients with dementia in this practice with a care review in the last 15 months.
DEM0004 - Spend by the NHS on providing memory assessment services per 1,000 people in the local population.
DEM0005 - Spend on anti-dementia drugs per 1,000 people, taking into account needs of the local population.
DEM0006 - Estimated percentage of people aged over 74 with dementia who had a social service review.
DEM0007 - Estimated percentage of people aged 65-74 with dementia who had a social service review.
DEM0008 - Estimated percentage of people aged 18-64 with dementia who had a social service review.
DEM0009 - Estimated percentage of people aged over 64 with dementia who received support services.
DEM0010 - Estimated percentage of people aged 18-64 with dementia who received support services.
DEM0011 - Percentage of people aged over 64 who were living at home 3 months after a stay in hospital.
DEM0012 - Percentage of people aged over 64 using social services who used self directed support or direct payments.
DEM0013 - Percentage of people over 65 receiving community services whose carer received support services.
DEM0014 - Number of key standards this hospital met in the National Audit of Dementia (2010).
DEM0015 - Number of Governance standards this hospital met in the National Audit of Dementia (2010).
DEM0016 - Number of Delivery of Care standards this hospital met in the National Audit of Dementia (2010).
DEM0017 - Number of Mental Health Need standards this hospital met in the National Audit of Dementia (2010).
DEM0018 - Number of Discharge Policy standards this hospital met in the National Audit of Dementia (2010).
DEM0019 - Number of Information standards this hospital met in the National Audit of Dementia (2010).
DEM0020 - Number of Recognition of Dementia standards this hospital met in the National Audit of Dementia (2010).
DEM0021 - Number of Training, Learning and Development standards this hospital met in the National Audit of Dementia (2010).
DEM0022 - Number of Specific Resources Supporting People with Dementia standards this hospital met in the National Audit of Dementia (2010).
DEM0023 - Number of Liaison Psychiatry standards this hospital met in the National Audit of Dementia (2010).
DEM0024 - Number of patients whose transfer of care was delayed at the end of a stay in this hospital.
DEM0025 - Percentage of patients with dementia who stayed in this hospital who were able to return to their 'usual place of residence'.

Data are provided in a standardised schema with each record containing an indicator ID and organisation ID. These can be matched against the Indicator Lookup, Organisation Lookup and Period Lookup tables included in the file, which also includes further metadata.
